Assessing liver stiffness with conventional cut‐off values overestimates liver fibrosis staging in patients who received the Fontan procedure

چکیده

Aim Patients who undergo the Fontan procedure for complex congenital heart disease are prone to liver cirrhosis. Liver stiffness (LS) reflects fibrosis stage in patients with chronic viral hepatitis; however, its accuracy predicting is controversial. We aimed clarify correlation between LS and patients. Methods Fifty-eight were prospectively measured transient elastography. undertook biopsy, cardiac catheterization, laboratory tests 22 of these (median age, 14.7 years; range, 9.9–32.1 years) > 11.0 kPa (median, 19.2 kPa; 12.2–39.8 kPa); elevated values suggest Results Histologically, all showed mild-to-severe portal sinusoidal but no Statistically, did not predict histological scores (p = 0.175). was correlated central venous pressure 0.456) or hepatic gradient (HVPG; p 0.062), although value HVPG only slightly above threshold significance. Conclusions developing both fibrosis. could be influenced by HVPG, using conventional cut-off overestimates overtreats

ARFI cut-off values and significance of standard deviation for liver fibrosis staging in patients with chronic liver disease.

BACKGROUND Acoustic radiation force impulse (ARFI) elastometry quantifies hepatic stiffness, and thus degree of fibrosis, non-invasively. Our aim was to analyse the diagnostic accuracy of ARFI cut-off values, and the significance of a defined limit of standard deviation (SD) as a potential quality parameter for liver fibrosis staging in patients with chronic liver diseases (CLD). MATERIAL AND...

Determining the progression stages of liver fibrosis in patients with chronic hepatitis B

Introduction: Chronic hepatitis B (CHB) leads to liver fibrosis, its failure, and death in the long term. The stage of fibrosis in CHB patients can also be detected based on the biochemical markers. The aim of this study was to predict the state of liver fibrosis in CHB patients and determine the possibility of patients shifting from a given state to another one. Materials and Methods: This stu...

Arterial stiffness in adult patients after Fontan procedure

OBJECTIVES Increased arterial stiffness is a risk factor of atherosclerosis and cardio-vascular complications. The aim of the study was to determine whether peripheral vascular function might be an early marker of impaired health status in patients with a single ventricle after Fontan procedure. METHODS AND RESULTS Twenty five consecutive adults (11 women and 14 men) aged 24.7 ± 6.2 years aft...

Biopsy-Controlled Liver Fibrosis Staging Using the Enhanced Liver Fibrosis (ELF) Score Compared to Transient Elastography

BACKGROUND AND AIMS Chronic liver diseases are characterized by inflammatory and fibrotic liver injuries that often result in liver cirrhosis with its associated complications such as portal hypertension and hepatocellular carcinoma. Liver biopsy still represents the reference standard for fibrosis staging, although transient elastography is increasingly used for non-invasive monitoring of fibr...

The combination of liver stiffness measurement and NAFLD fibrosis score improves the noninvasive diagnostic accuracy for severe liver fibrosis in patients with nonalcoholic fatty liver disease.

BACKGROUND & AIMS The accuracy of noninvasive tools for the diagnosis of severe fibrosis in patients with nonalcoholic fatty liver disease(NAFLD) in clinical practice is still limited. We aimed at assessing the diagnostic performance of combined noninvasive tools in two independent cohorts of Italian NAFLD patients. METHODS We analysed data from 321 Italian patients(179 Sicilian-training coho...
